Guest Check-In - Wonder Play Cafe

I, the undersigned parent, legal guardian, caregiver, event host, or responsible party, acknowledge and agree to the following Guest Check-In and Safety Acknowledgment on behalf of myself, my child(ren), and all guests included in my reservation, visit, party, or event at Wonder Play Cafe.

This includes, but is not limited to, spouses, adult guests, friends, caregivers, minors, and any other attendees in my party.

I understand that participation in indoor play, activities, classes, events, and use of the Wonder Play Cafe space involves certain inherent risks, including but not limited to falls, slips, collisions, contact with other guests, contact with play structures, or other accidental injuries.

I voluntarily assume all risks associated with my participation, my child(ren)’s participation, and the participation of any guests in my party.

I understand that Wonder Play Cafe is not a childcare facility. Children must be supervised by a parent, legal guardian, caregiver, or responsible adult at all times.

I agree that I am responsible for supervising my child(ren), any children in my party, and any guests included in my reservation, visit, party, or event. I agree to help ensure that all guests follow posted rules, staff guidance, and safety guidelines while inside Wonder Play Cafe.

I understand that Wonder Play Cafe may offer or allow food and beverages on site. I am responsible for notifying Wonder Play Cafe staff of any known allergies, sensitivities, or dietary restrictions for myself, my child(ren), or any guests in my party.

I understand that Wonder Play Cafe will take reasonable measures to support guest safety but cannot guarantee an allergen-free environment.

I acknowledge that alcohol may be served or consumed during certain private events or approved experiences at Wonder Play Cafe.

I understand that Wonder Play Cafe expects all guests to consume alcohol responsibly. Any unsafe, inappropriate, or irresponsible behavior related to alcohol may result in removal from the premises without refund.

To the fullest extent permitted by law, I release and hold harmless Wonder Play Cafe, its owners, employees, representatives, partners, vendors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from participation in activities, events, open play, classes, or use of the premises.

This includes, but is not limited to, personal injury, illness, property damage, loss, or any incident involving myself, my child(ren), or any guests in my party.

I understand that I, or another responsible adult in my party, must remain present during my child(ren)’s participation at Wonder Play Cafe.

In the event of a medical emergency, I authorize Wonder Play Cafe staff to contact emergency services and seek necessary medical assistance for myself, my child(ren), or any guests in my party.

I understand that Wonder Play Cafe staff may provide reasonable assistance in an emergency until professional medical help arrives, and I release Wonder Play Cafe from liability related to good-faith emergency assistance, to the fullest extent permitted by law.

I understand that photos or videos may be taken at Wonder Play Cafe for promotional, social media, website, or marketing purposes.

By completing this check-in, I grant Wonder Play Cafe permission to use photographs or videos of myself and/or my child(ren) for promotional purposes, unless I notify Wonder Play Cafe in writing that I do not consent to photography or publicity use.

I acknowledge that I have read, understand, and voluntarily agree to the terms of this Guest Check-In and Safety Acknowledgment.

I understand that this check-in applies to myself, my child(ren), and all guests included in my reservation, visit, party, or event at Wonder Play Cafe, including adult guests, spouses, friends, caregivers, minors, and any other attendees in my party.

I further acknowledge that this Guest Check-In and Safety Acknowledgment will remain in effect for all future visits to Wonder Play Cafe unless updated or replaced.

By providing my contact information and submitting this form, I confirm that I am the responsible party for my reservation, visit, party, or event, and I agree to all terms listed above.
